Not able to integrate new template

This is very basic plugin. It doesn’t works for the custom fields. Also Clio Grow custom template will not support any simple html embed code as well. So I do not recommend to use Clio Grow for lead collection.

This is a disappointing plugin. My clients, lawyers who use the Clio CRM and Clio Grow, wanted to use this form so that when their clients completed a contact form on their website the information would be integrated into their Clio CRM.
Unfortunately, the options are few for capturing other than basic information and customizing it for other than the basic WordPress themes is difficult.
The plugin is not tested with newer versions of WordPress and doesn’t appear to be updated regularly.
Disappointing.
